2) Updated Bank Passbook With a Letter on Bank Letter Head Mentioning Name, Resident Address with Duration. (present address should be for minimum 1 year)

For Age Proof :(any one from below)
1) Birth Certificate / School or College Leaving Certificate / SSC Diploma
2) Affidavit if Applicant s 55 year or above and is Unable to Produce any Other Document.

For Emigration Clearance (optional) (any one from below) :
1) Matriculation or above Passing Certificate.
2) Income Tax Payment Challan (saral) for Last 03 consecutive years.

Marriage Proof :
Marriage Certificate or Affidavit as per Prescribed format.

Photograph :
Passport Size (35mmx45mm) COloured - 06 Nos. Straight Front Face with Plan Light Background.

For Govt. Employee :
NOC or Intimation Letter

For Renewal Of Old Passport :
Old Passport with Recent Photograph (address / ELNR / marriage document if change form old passport)

For Minor Applicant (below 18 years) :
1) Passport of Mother & Father
2) Birth Certificate from Municipal Corporation or Date of Birth Letter from School on School Letterhead.
3) Photograph - 06 Nos.

Important :
1) All Documents Required in Original Along with two Xerox Copy and Signed Application form.
2) Original Document Will be Returned after Verification by Passport Office at Time of Submission of Passport Application.
3) Validity of Passport : Adult 10 years, Minor 05 years.
